When the consumer contacts the Head to Health telephone line (1800 595 212), they will be asked to enter their postcode, and for South West, Central West and North West Queensland, it will be routed to the WQPHN’s Head to Health Phone Service Provider, RHealth.
A trained worker will go through the Initial Assessment and Referral Guidance and Decision Support Tool (IAR DST) with a consumer or work with the caller to identify useful pathways to needed supports. For consumer callers, the IAR Guidance and Decision Support Tool (IAR DST) is a holistic decision-making framework that assists support workers and clinicians in matching consumers with the most appropriate mental health services for their needs.
Over the past three years, the Health Service Navigator program has provided mental health navigation services across the Darling Downs and West Moreton regions. These navigation services are now transitioning to the new national service, Head to Health. Head to Health will continue to provide free and confidential support for anyone seeking help for their wellbeing or someone they care about. The Head to Health telephone line 1800 595 212 is already up and running to provide support for individuals and service providers across our region.

headspace Warwick is a health service for those aged between 12 and 25 years, providing support for general health, mental health, drug and alcohol issues, and employment and educational support. The focus of headspace Warwick is on early intervention and having a one stop shop approach to health issues.
RHealth is the Lead Agency of headspace Warwick.
The headspace Warwick office is located in Warwick and provides outreach services to Stanthorpe.

Care Coordinators offer support to young people by helping put together a tailored care plan for each client. The Care Coordinator is there to help guide and support a young person through the headspace experience and advocates on their behalf in and out of the headspace environment.
There are appointments available within headspace Warwick with a variety of other Mental Health Professionals (eg Psychologist, Psychiatrist, Counsellors), if a need is identified with a young person.

RHealth has been managing the Rural Primary Health Service (RPHS) program, on behalf of the Central Queensland, Wide Bay, Sunshine Coast Primary Health Network (CQWBSC PHN), since 2015. This program operates in the Wide Bay region with a focus on the North Burnett.
The Rural Primary Health Service program allows those living with a Chronic Disease to access a variety of Allied Health Professional services. RHealth works closely with the regions GPs and the Wide Bay Health & Hospital Service to provide a co-ordinated, multi-disciplinary based approach to each client’s health care needs, encouraging people in rural and remote areas to adopt or modify behaviours to better manage their health and wellbeing.
RHealth also provides high-quality data collection, management and reporting services for this program to the CQWBSC PHN, to ensure services are being provided for those most in need in the rural and remote areas of the Wide Bay.
