RHealth has over 24 years of experience in Project & Program Management. In design and maintenance of effective Projects & Programs, RHealth has a great understanding of how to engage in the local needs and operating context of the communities we work with.

Projects & programs delivered by RHealth are managed using best practice systems including PRINCE2 methodology and quality accredited processes. Our stakeholders can have a high degree of confidence that RHealth will deliver projects on time, on budget, and within scope. Features

Referral tracking

GP's and referrers can track the progress of referrals entered into the system using several different methods, allow them to ensure their patients are being seen and are received the best care possible.

Data Quality Management

RHealth manages the data that is entered into refeRHEALTH, regularly conducting quality checks to ensure that it is accurate. Data validation is built into the workflow so users can spend less time on checking their data and more time on delivering services.

Invoicing

refeRHEALTH has a built-in invoicing and budget module that allows service providers to generate invoices directly from session data they have entered.

Notifications and Messaging

refeRHEALTH has a built in ‘Notes’ features that can be used as an internal messaging and as a system for additional records. This system allows service providers or referrers to notify both each other or refeRHEALTH administrators without the need to call or email.

Reporting

RHealth is able to provide comprehensive reports from the data that is collected in refeRHEALTH beyond standard MDS reports, including reports on demographics, referral data including referral source, session data and outcome measures.

Security

refeRHEALTH is secured through multiple methods by RHealth. All data is stored on cloud servers based in Australia, and secured and managed by an Australian based organisation.

What are Health Service Navigators

The Health Service Navigator is a free service that can assist community members and service providers to navigate local pathways to non-urgent mental health, wellbeing and other community support services, in the Darling Downs and West Moreton region.

RHealth has Health Service Navigators working across the Western Downs, Taroom, Southern Downs, Goondiwindi, South Burnett, Cherbourg and Somerset regions.

Neami National is the provider for the Health Service Navigator Program for Ipswich, Toowoomba, Scenic Rim and Lockyer Valley regions.

How can Health Service Navigators help?

Health Service Navigators work collaboratively with community members, General Practitioners and other mental health care and community service providers, to ensure the most appropriate information is provided to support a person’s wellbeing journey.

Accessing the service

This service is available to individuals and service providers within the Darling Downs and West Moreton PHN region of Queensland.

When accessing support from a Health Service Navigator, people can be assured of a confidential, person-to-person, free service that will assist with finding mental health and community supports to meet their needs.

Health Service Navigators do not provide an emergency response, make home visits nor undertake client case management.

Anyone seeking assistance from a Health Service Navigator can call 1300 012 710 between 9:00am and 5:00pm weekdays.

Health professionals may also be able to make formal referrals via refeRHEALTH.

headspace Warwick

headspace Warwick

headspace Warwick is a health service for those aged between 12 and 25 years, providing support for general health, mental health, drug and alcohol issues, and employment and educational support. The focus of headspace Warwick is on early intervention and having a one stop shop approach to health issues.

RHealth is the Lead Agency, and as part of the consortium works in partnership with Drug-Arm, Darling Downs Hospital and Health Service District, Southern Downs Regional Council, Artius and Southern Queensland Institute of TAFE.

The headspace Warwick office is located in Warwick and services the Southern Downs region.

Care Coordinators offer support to young people by helping put together a tailored care plan for each client. The Care Coordinator is there to help guide and support a young person through the headspace experience and advocates on their behalf in and out of the headspace environment.

Mental health services

There are appointments available within headspace Warwick with a variety of other Mental Health Professionals (eg Psychologist, Psychiatrist, Counsellors), if a need is identified with a young person.

headspace Roma

headspace Roma

headspace Roma is a health service for those aged between 12 and 25 years, providing support for general health, mental health, drug and alcohol issues, and employment and educational support. Rural Primary Health Service - Widebay

Rural Primary Health Service

RHealth has been managing the Rural Primary Health Service (RPHS) program, on behalf of the Central Queensland, Wide Bay, Sunshine Coast Primary Health Network (CQWBSC PHN), since 2015. This program operates in the Wide Bay region with a focus on the North Burnett.

The Rural Primary Health Service program allows those living with a Chronic Disease to access a variety of Allied Health Professional services. RHealth works closely with the regions GPs and the Wide Bay Health & Hospital Service to provide a co-ordinated, multi-disciplinary based approach to each client’s health care needs, encouraging people in rural and remote areas to adopt or modify behaviours to better manage their health and wellbeing.

RHealth also provides high-quality data collection, management and reporting services for this program to the CQWBSC PHN, to ensure services are being provided for those most in need in the rural and remote areas of the Wide Bay.

RHealth is a proven provider in event management. From small events for staff training to large conferences, RHealth has the skills and knowledge to bring your event to life. We listen to what you require, what your deliverable’s are, what your key message is and who your target audience is. RHealth will tailor your event to your specific needs and can meet with you before you commit to discuss options. RHealth will work with your staff and managers to ensure the event is delivered on time, seamlessly and to get as much value for money, as we are not for profit we exist to help you deliver your best. We can support your own staff with an event they are running or manage the whole event and run it for you.

RHealth is also committed to delivering quality education sessions and courses to GP’s and their staff. RHealth is selective in what education training is delivered so that GP’s and health professionals, because of this our clients know they will get quality training.
