Seeking a Mental Health Nurse – 2-3 Days per week

RHealth is seeking to contract a Mental Health Nurse or a Registered Nurse with mental health experience for 2-3 days per week to deliver a service into the South Burnett and Cherbourg region.

This service is funded under the Mental Health Nurse Care Program by the Darling Downs and West Moreton Primary Health Network until the 30 June 2019.  This contract would be flexible, rather than fixed hours on fixed days.

The service is for people experiencing severe and complex mental illness who are currently being managed in the primary care setting via a General Practitioner (GP) and/or a Psychiatrist. More specific information can be found here Service Specifications - Mental Health Nurse Care Program It is delivered as a mix of clinical intervention and care coordination, connecting clients into other community services that would benefit their recovery. This service is free to patients who are eligible.

PIR Support Facilitator Warwick

Do you want to make a difference supporting people in the Southern Downs needing mental health services to complete the NDIS transition for PIR ? 

The Partners In Recovery (PIR) Program is funded by the Australian Government to improve outcomes for people experiencing severe and persistent mental illness. The Support Facilitator engages with assigned participants to support and facilitate their transition to the NDIS, including assistance with the NDIA application, assessment and planning processes where requested by the client. The Support Facilitator also transitions PIR participants who are not eligible for the NDIS to appropriate alternative supports before the end of the NDIS transition phase.

Based in the Warwick Office, travel throughout the region is required. The PIR Support Facilitator reports to the RHealth Programs Manager and is accountable for administration and reporting.

To make an impact you will have experience and qualifications in human services, social work or similar, particularly in mental health programs.  A driver’s license, confidence in driving long distances, and program administration skills are required for the position.
