What are Health Service Navigators?
The role of a Health Service Navigator is to:
- Assist GPs, Service Providers, Consumers and Carers to navigate options for appropriate mental health services in your area;
- Coordinate mental health supports and services for Consumers and their Carers;
- Map and identify health service needs and gaps;
- Provide information about services funded by the Darling Downs and West Moreton PHN and other community providers and programs.
How can Health Service Navigators help you?
Health Service Navigators are community service professionals who can support you in understanding and navigating the pathways to accessing mental health services within a stepped care approach. This process is often challenging and confidential assistance can be of particular help to people who are struggling with their well-being.
Health Service Navigators can act as your first point of call to evaluate what mental health supports are available in your region and how that support could be funded. The Health Service Navigator works with people to find the right service, with the right person at the right time and can assist people at all stages of their mental health journey.
Referrals are accepted into RHealths Health Service Navigator Program from a broad range of referral pathways including GPs, Allied Health Professionals, Carer’s and Self-Referrers.
Accessing the service
Anyone seeking assistance to better their mental health and well-being can call RHealth on 1300 012 710 between 9:00am and 5:00pm weekdays to speak with their local Health Service Navigator.
RHealth has Health Service Navigators engaged in the Western Downs, Southern Downs, Goondiwindi, South Burnett, Cherbourg and Somerset regions.
Neami National is the provider for the Health Service Navigator Program for Ipswich, Toowoomba, Scenic Rim and Lockyer Valley regions.
Making a referral to the Health Service Navigator Program
Referrals by GPs and Primary Health providers can be made using the refeRHEALTH e-referral tool or by using the forms available on the DDWMPHN website and emailing or faxing the relevant form to either firstname.lastname@example.org or 07 4638 2499
Carer’s, Community Members and Individuals can call the Health Service Navigators directly on 1300 012 710
This program is funded by the Darling Downs and West Moreton Primary Health Network under the Australian Government’s PHN Program and is jointly delivered by RHealth and Neami National.
