What are Health Service Navigators?
The Health Service Navigator is a free service that can assist community members and service providers to navigate local pathways to non-urgent mental health, wellbeing and other community support services, in the Darling Downs and West Moreton region.
RHealth has Health Service Navigators working across the Western Downs, Taroom, Southern Downs, Goondiwindi, South Burnett, Cherbourg and Somerset regions.
Neami National is the provider for the Health Service Navigator Program for Ipswich, Toowoomba, Scenic Rim and Lockyer Valley regions.
How can Health Service Navigators help?
Health Service Navigators work collaboratively with community members, General Practitioners and other mental health care and community service providers, to ensure the most appropriate information is provided to support a person’s wellbeing journey.
Accessing the service
This service is available to individuals and service providers within the Darling Downs and West Moreton PHN region of Queensland.
When accessing support from a Health Service Navigator, people can be assured of a confidential, person-to-person, free service that will assist with finding mental health and community supports to meet their needs.
Health Service Navigators do not provide an emergency response, make home visits nor undertake client case management.

This service is funded by the Darling Downs and West Moreton PHN, under the Australian Government’s PHN Program, and is jointly delivered by RHealth and Neami National.
