The Queensland Health Connecting Healthcare In Communities (CHIC) Initiative started in 2007, establishing Partnership Councils in each Health Service District (16 at the time):
- RHealth is a member agency of the South West, West Moreton South Burnett, Toowoomba & Darling Downs and Southside Partnership Councils
- Coordinators for the South West and West Moreton South Burnett Partnership Councils are employed by RHealth
- Partnership Councils link primary health care providers together to create better local systems for chronic disease care
- Local Projects funded through specific Service Delivery & Innovation and Chronic Disease funds support this work from funds allocated by Queensland Health (Southern Area) to each Partnership Council
- Reducing duplication of services and addressing service gaps through a coordinated approach to delivery
