Better coordinated patient care for Type II Diabetes is the aim of the South Burnett Health Education Project. As part of a larger Project implemented in partnership with Ipswich West Moreton Division of General Practice, RHealth works towards this by:

• Identifying health professionals interested in undertaking a Graduate Certificate in Diabetes Education
• Supporting health professionals to become credentialed Diabetes Educators
• Identifying health professionals to become Diabetes Resource Workers
