Better coordination of patient care is the aim of the Australian Better Health Initiative Primary Care Incentive Program (ABHI). RHealth works with health professionals in the South West region, including the towns of Charleville, Roma, St George, Quilpie, Mitchell, Dirranbandi and Surat to support chronic disease patient care through:

• Improved communication between health professionals, and with patients
• Raising awareness of service availability
• Developing care pathways and referral tools
